Thinking of Sagan’s comment, St. Augustine said (sometime during the late 4th or early 5th century) that he was astonished to discover it was possible to read without speaking the words aloud. I wonder when reading “in our heads” became common and whether it solidified the now-familiar effect Sagan speaks of.

That is an excellent point, and I believe reading to oneself became more common in the Renaissance after the creation of the printing press. This allowed for the proliferation of books, which brought their prices down, allowing many more people to both learn to read and to read for pleasure.
